What to Expect on Your First Visit

Welcome to NWA Health Solutions!! First off, we would like to congratulate you on your choice to take control of your health! We are excited to work with you on this journey and exploration of a healthy lifestyle.

On this page, you will find the New Patient Paperwork.  Please print, fill out, and bring with you to your first visit.  For a child, please use the Pediatric Paperwork.  Arrive about 5 minutes early, so our staff can get your information entered into the computer and file assembled for you.

​

Please allow 30-45 minutes for your first visit at NWA Health Solutions.  Upon completion of your first visit, we will get you rescheduled for your second visit to discuss treatment plan options for you or your child.

Statute of Limitations Chapter 56 of the Arkansas legal code includes a subsection dealing specifically with medical bills. Prior to April 1985, medical practitioners and providers had just 18 months to collect past due debts. For debts incurred after that date, the statute of limitations has been extended to two years. As with written contracts, the statute of limitations is reset whenever you make a partial payment. The statute could be extended for several years if you enter into a periodic payment agreement.
